In the sections where we asked for medical and disability details, please tell us anything which you consider it would helpful for us to know, even if not what you would call a medical issue (eg. very shy, neurodivergent).

In submitting this form, you agree:

• to update the club without delay of any changes to your contact details;
• to pay fees monthly in advance by standing order unless other arrangements are made and to pay any other charges in full on demand; and
• that the Club may send you documents and information in electronic, rather than physical, form.

Help from parents/ guardiansThe club cannot exist without help from volunteers. If you could indicate in the "extra information" box what help you will be able to offer, we'd be very grateful. Examples are:

• helping poolside at swim meets
• training as an official or timekeeper
• joining the committee
• promoting and marketing
• updating the noticeboard
• other admin support
• fundraising
• organising social events

The club may wish to take photographs of individuals and groups of swimmers under the age of 18 which may include your child. All photographs will be taken and published in line with the Swim England Photography Policy. The club requires parental consent to take and use photographs. Parents have a right to refuse agreement to their child being photographed. As the parent or carer please indicate your permission below. Please note you can withdraw your consent in writing to the club Welfare Officer at any time should you wish to do so.
